The Support at Home Program is part of the Australian Government’s aged care reform, aimed at creating a simpler, fairer, and more consistent system for older Australians. This program will combine existing in-home care services into one streamlined model—helping older people stay safe, independent, and connected in the comfort of their own homes.

What is the Support at Home Program?
The Support at Home Program is being introduced by the Federal Government as part of reforms recommended by the Royal Commission into Aged Care Quality and Safety. It will officially commence on 1 July 2025.
This new system will:
- Combine the existing Home Care Packages, Commonwealth Home Support Programme (CHSP), and other in-home aged care programs.
- Make it easier to access care, equipment, and home modifications.
- Provide more flexibility in how funds can be used.
- Ensure older people receive the right level of support to stay active, healthy, and socially connected.

What Does This Mean for You or Your Loved One?
If you currently have a Home Care Package (HCP) or are waiting for one, you don’t need to take any action. Starting 1 July 2025, you will automatically move to the Support at Home Program. Your current funding level will remain the same, and any unspent funds will carry over for continued use on approved services or equipment.
If you received your Home Care Package before 12 September 2024, you are considered a Grandfathered Participant. This means your transition will be seamless, and you will not pay more than you currently do under your existing package.
If you are applying for care or awaiting assessment, you can still start the process through My Aged Care. Anchor Care can assist you in understanding the steps and preparing for your assessment.
Key Changes Under the New Program
1. Classification Levels
The Support at Home Program will feature eight care levels (replacing the current four). This allows for more precise funding tailored to individual needs.
2. Care Management
Ten percent (10%) of each participant’s budget will be allocated to care management—ensuring consistent coordination and quality oversight of your care services.
3. Assistive Technology and Home Modifications
Participants may receive additional funding (ranging from $500 to $15,000) for approved assistive technology or home modifications to support independent living. Anchor Care will help you access and manage these supports as part of your care plan.
4. Short-Term Care Pathways
Two new pathways will be introduced:
- Restorative Care Pathway: Short-term allied health and support services (up to 12 weeks) for recovery and independence.
- End-of-Life Pathway: Urgent support for people with a terminal illness, offering funding up to $25,000 for in-home care and services.
