Services for people under 65 in Victoria.

Care Connect has more than 25 years of experience in supporting people with disability, chronic illness or short-term health needs to remain independent, at home and in the community.

Victorian residents aged under 65, or under 50 for Aboriginal and Torres Strait Islander people, may be eligible to access our Home and Community Care Program for Young People (HACC-PYP) Linkages program.

Linkages Program.

The HACC-PYP Linkages program is available to Victorians aged under 65, or under 50 for Aboriginal or Torres Strait Islander people, who have complex care needs and would benefit from case management support.

Linkages Program. The HACC-PYP Linkages program is available to Victorians aged under 65, or under 50 for Aboriginal or Torres Strait Islander people, who have complex care needs and would benefit from case management support. People may be eligible if they require assistance with daily activities due to physical and/or psychosocial functional impairment related to disability, chronic illness or short-term health needs. This assistance might include personal care, dressing, preparing meals, house cleaning, property maintenance, community access, or using public transport.

Care Connect manages HACC-PYP Linkages packages in the inner and outer Eastern and Western metropolitan regions of Victoria.

A dedicated Linkages case manager will access a range of supports based on a persons goals, aspirations and needs, to improve or maintain their capacity to remain living independently at home, and in the community. This may include services such as:
