Clinical Care at Home – Safe, Skilled, and Compassionate

Nursing and Chronic Condition Management

Under the Support at Home program, launching on 1 November 2025, Category 3: Health & Specialised Support includes clinical services delivered by qualified health professionals to help older Australians manage complex health needs at home.

Our model includes:

  • Nursing Care: Delivered by qualified nurses, focusing on medication management, wound care, health monitoring, and chronic condition support.
  • Clinical Oversight: Ensuring care is provided within a clinical governance framework, with appropriate escalation and supervision.
  • Restorative and Reablement Support: Helping clients recover from illness or injury and regain independence.

These services are designed to reduce hospital admissions and support older Australians to remain safely at home. Under the new Aged Care Act, clinical care must be delivered in a way that is safe, evidence-based, and person-centred, with clear protocols for escalation and coordination.

Reducing Hospital Admissions Through Home-Based Care

Clinical care at home is not just about convenience—it’s about preventing unnecessary hospitalisation and supporting better health outcomes. Our future service model is designed to:

  • Monitor chronic conditions such as diabetes, heart disease, and respiratory illness.
  • Provide timely interventions to prevent deterioration.
  • Support medication adherence and safe use of medicines.
  • Coordinate with GPs, allied health professionals, and hospitals when needed.

We are aligning our systems and workforce with the Strengthened Aged Care Quality Standards, which require:

  • Qualified staff working within their scope of practice.
  • Clinical governance integration into corporate governance.
  • Monitoring and reporting of adverse events, including medication-related incidents.

Our goal is to deliver clinical care that is not only compliant but also compassionate, empowering, and responsive to each client’s health journey.

  • Embedding the Statement of Rights into all clinical interactions and service agreements.
