Effective aged care goes beyond delivering services—it’s about understanding each person’s unique needs, goals, and preferences. Under the Support at Home program, Category 5: Care Management focuses on planning, coordination, and continuous review to ensure care remains relevant and responsive.

Our care management model includes:

  • Care Planning: We develop a tailored care plan that evolves with changing circumstances, ensuring services remain aligned with the client’s needs.

These practices are aligned with the new Aged Care Quality Standards, which require providers to deliver person-centred, culturally safe, and rights-based care. Our approach ensures that clients are active participants in their care journey, not passive recipients.

Ongoing Monitoring & Service Coordination

Care needs change over time, and our role is to ensure that services adapt accordingly. Under the new Act, care management includes:

  • Coordination: Ensuring services are delivered seamlessly across providers, including allied health, nursing, and social support.
  • Review: Regularly monitoring progress and adjusting support when required.
  • Advocacy & Support: Helping clients navigate the aged care system, access additional services, and make informed decisions.

We are preparing to meet the digital reporting and compliance requirements of the Support at Home program, including:

  • Use of the Government Provider Management System (GPMS) for care documentation and updates.
  • Integration of the Statement of Rights into all care planning and client communications.
  • Transparent service agreements that reflect client preferences and ensure accountability.

Our care managers will be trained to uphold the principles of supported decision-making, ensuring that clients retain control over their care and are empowered to make choices that reflect their values.
